Working across the Medway area, you will provide one-to-one support, develop recovery-focused support plans, complete risk assessments, liaise with partner agencies, and help tenants access the services they need to thrive. You will also play a key role in crisis intervention, safeguarding, tenancy sustainment, and promoting social inclusion.
We are looking for someone with experience of supporting people with mental health needs, excellent communication skills, strong organisational abilities, and a positive, non-judgemental approach. Experience of support planning, risk assessments, partnership working, and maintaining accurate records is desirable. A car owner/driver is essential due to travel across Medway.
